FCAA Mail-In Instructions

1.    Print off the Registration, Coach or Umpire Form.

2.    Complete all fields and sign where indicated.

Player Registration: Medical Release page MUST be completed and signed.  If you are not insured through a medical plan, please state this.  No child can play without the Medical Release signed by a parent or guardian.

3.    Mail the form along with a check or money order to “FCAA” to:

There is no fee to register as a Coach/Umpire – Mail in the form only.

4.    All Player and Head Coach Mail In Registration Forms MUST be postmarked no later than January 30^th.  Assistant Coach and Umpire registration will be accept until March 1^st.

5.    Once your form is received by the FCAA, you will receive a confirmation email or letter.  We strongly encourage the use of email as a cost saving measure.
