FCAA Player Mail-In Registration Instructions

1.     Print off the Player Registration Form and complete in full. 

2.   The Medical Release page MUST be completed and signed.  If you are not insured through a medical plan, please state this.  FCAA is required to obtain this information for each player.  No child can play without the Medical Release signed by a parent or guardian.

3.    Incomplete forms will result in the child not being registered for the upcoming ball season and could result in a late fee being imposed.  Any areas of the form not addressed will require follow up with the parent/guardian for completion.

4.   When complete, the form along with a check or money order made payable to “FCAA” can be mailed to:

5.     All Mail In Registration Forms MUST be postmarked no later than January 25^th.

6.   Once your form is received by the FCAA, you will receive a confirmation letter along with 12 raffle tickets by mail for each child who registers.  The money from the sale of the tickets can be kept by the individual who sells the tickets which is a cash back bonus to you on your registration fee already paid.

Registration Fees 2008

·        One child - $55

·        Two children - $95

·        Three children - $120

·        Four or more - $140

(Late Fee - $15.00 applies to forms postmarked after 01/25/08)
